needs of the individuals or families in the program.
This could include SROs or multiple bedroom apartments. |

-Rental assistance
-24-hour emergency staff coverage (residents have a
beeper number)
-Case management (generally weekly home visits)
-Services provided at Community HealthLink include:
Substance use counseling
Mental health counseling
HIV education |

LEASING INFORMATION: |
Community HealthLink subleases 30 units (subsidized
through Shelter+Care) to residents. An additional 36
units (subsidized through a different funding source)
are leased through private landlords. |
