RESIDENTS SERVED: 64 Men & Women or Families

Serves 64 men and women living with HIV/AIDS or families with at least one adult member living with HIV/AIDS. Applicants must be homeless. Applicants taking methadone are eligible for this program. HOAP also serves 12 persons with mental illness.

SETTING: HOAP consists of 64 scattered-site apartments based on the needs of the individuals or families in the program. This could include SROs or multiple bedroom apartments.

-24-hour emergency staff coverage (residents have a beeper number)
-Case management (generally weekly home visits)
-Services provided at Community HealthLink include:
Substance use counseling
Mental health counseling
HIV education
LEASING INFORMATION: Community HealthLink subleases 30 units (subsidized through Shelter+Care) to residents. An additional 36 units (subsidized through a different funding source) are leased through private landlords.
